Office Records Retention Schedules
Office-level retention schedules apply to records created and maintained by, and usually unique to, individual offices. Not all offices use or need these specific schedules. In any case, staff will always need to consult the WWU General Schedule for instructions regarding common types of records that are not unique to an office.
Titles on office retention schedules are not transferrable to other offices. If your office needs specific titles on its office-level schedule (or would like to create an office schedule if one does not exist), contact UARM.

Confidentiality Agreements
Confidentiality agreements signed by employees and students of the Communication Sciences and Disorders Department.
Graduate School--Comprehensive Exams
Includes comprehensive exam taken by graduate students as part of the requirements for graduation in the Speech Pathology/Audiology program. Final test scores maintained in student files.
Graduate Student Files (Individual Student Files)
Provides documentation of clinical practicum experience/clock hours and evaluation for audiology and speech pathology upper division and graduate students. Graduate student files include copies of application, transcript, test scores, & related documents.
Personal Health Information Privacy and Security Training Completion Records
Records documenting employees and students who have completed the Communication Sciences and Disorders Department required PHI privacy and security training.
Personal Health Information Privacy and Security Training Course Materials
- Materials presented (PowerPoint slide deck, handouts, etc.)
- Attendee lists/sign-in sheets documenting who attended
- Tests administered and results
- Course/seminar/workshop evaluation feedback/surveys
- Certification/hours/credits/points awarded (if applicable).
